GRIL LIABILITY WAIVER / AUTHORIZATION RELEASE
In consideration of being allowed to participate in this event sponsored by GRIL and its partners and in consideration of the benefits to be derived there from, I hereby release GRIL and its present and former trustees, officers, directors, shareholders, employees, agents, and their heirs, administrator, executors, successors, and assigns from all claims and liabilities of any kind, whether known or unknown, which arise from or are connected in any way with my/ my child’s participation in this trip.
I recognize that the conditions in some of the places to which I/my child will travel are not of the same standard as the conditions to which I/they are accustomed, I realize further that there are certain health risks as well as other risks I/my child will be exposed to while participating on this project. I fully acknowledge those risks. If for any reason I/my child am/is unable to complete my/his/her stay at the project, I assume full responsibility for the expenses incurred for my/his/her return home.
I am also aware that any lack of respect for authority, including the use of abusive language, violations of site smoking policies, and failure to comply with behavioral expectations may result in my/his/her being sent home at my expense. I understand that any use or possession of alcohol rearms, weapons, or drugs (not prescribed by a physician) will automatically result in my/his/her being sent home at my expense.
In the event of an emergency, I hereby authorize a leader of this activity, as an agent for me, to consent to: any x-ray examination, medical, dental, or surgical diagnosis; treatments; hospital care advised and supervised by a physician, surgeon or dentist (as appropriate) licensed to practice under the laws of the state or country where services are rendered, either at a doctor’s office or in a hospital. I expect my emergency contact/I will be contacted as soon as possible.
I understand that this document constitutes a full and complete waiver of all possible claims, including claims of negligence, arising out of my and/or my child’s participation in the trip. I also agree that I will defend, indemnify, and hold harmless the parties released from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney fees, brought as a result of my or my child’s participation in the trip, even if the damages or injuries are caused in whole or in part by the released parties.
I understand that this release applies to, covers, and includes unknown, unforeseen, unanticipated, and unsuspected damages, losses or liabilities and the consequences thereof, which result from the matters herein before inferred to as well as those now disclosed and known to exist. The provisions of any state, federal, local, provincial, or territorial law or statute providing in substance that releases shall not extend to claims or damages which are unknown or unsuspected to exist at the time are hereby expressly waived by me.
ALL YOUTH REGISTRANTS & PARENTS / GUARDIANS
I certify that all information in this application is correct. I have read, understood and agreed to all the provisions of the Liability Waiver / Authorization Release. I promise to participate fully in the event, have read the event rules and agree to abide by them. I also give permission for my child to be photographed for use in GRIL / Where U At? promotional materials including, but not limited to, printed brochures and web site photos / promotion. My child’s image will not be used for any other purpose that the promotion of GRIL / Where U At?.
ALL ADULTS
GRIL requires that all Adult Leaders and Host Team members have a current Background Check on file at their church / ministry office. It must be a national (multi-state/province) criminal background check and sex of fender registry check.

I understand that by participating in this event, I am establishing myself as a role model for the youth who attend. I promise to participate fully in this event, obey the rules. Further I will assist event leadership by encouraging like conduct among the youth. My image will not be used for any other purpose than the promotion of GRIL / Where U At?.0 I do hereby certify that I promise to abide by the rules and regulations set forth therein.
I certify that this adult is capable of leading a small group of youth and will represent our church / ministry as a superior Christian role model for the youth at Where U At?. I certify that this adult, to my knowledge, has strong moral character and has no criminal record related to violence or abuse of any kind. I also certify that this adult has a Background Check on file with our church / ministry.

YOUNG LIFE WAIVER
AUTHORIZATION FOR TREATMENT
I/We hereby give permission to the medical personnel selected by Young Life to secure and administer treatment and to maintain and/or release any medical records necessary for insurance purposes to provide or arrange necessary related transportation for the above named person.
I have received notice and agree to the release of my personal information and required health information as outlined in Young Life’s “Privacy Statement.” Available at https://www.younglife.org/privacy-policy/.
INDEMNITY AND CONTRACT AGREEMENT
I have been informed of the risks related to this Event, including any inherent risks that cannot be eliminated, and I expressly assume any and all risks of injury, illness or infectious and/or contagious diseases or sicknesses, death, or damage to my person or property arising from or relating to my or my child’s participation in the Event, including travel to and from the Event. In doing so, I acknowledge and certify that I, or my child, am/is
fit to participate in this Event and have not been advised otherwise by a qualified medical person. I recognize that my/my child’s participation in the Event is a privilege and as consideration for this privilege, I waive and release any and all actions, claims, suits or demands of any kind or nature whatsoever against Young Life, its corporate affiliates, contractors, vendors, officers, agents, sponsors, volunteers or representatives of any kind (collectively “Releasees”) arising from or relating in any way to my/my child’s voluntary participation in the Event, including travel to and from
the Event and the Activities at the Event, unless arising out of the willful or grossly negligent act of the Releasees, and I agree to indemnify the Releasees should any such loss, damage or claim occur.
I verify that I/my child named is in good health and capable of participating in strenuous activities and, when necessary, will tailor my/their activities to those within the bounds of my/their physical health. I further acknowledge that I have had an opportunity to inquire about the kinds of activities which will take place at the Event and that I/my child am capable of participating in all activities except what I indicated in the
limitations to activities section above. I understand that participation in Young Life activities requires the use of certain safety equipment. I have been provided information on the safety equipment that I, or my child, will be required to wear at all times during the specific activity. I understand that I, or my child, assumes responsibility for inspecting the safety equipment prior to its use to ensure that the safety equipment is in good condition and to timely notify Young Life if there are any concerns prior to using the safety equipment.
I understand that signing this Consent/Release means, among other things, that if I am/my child is injured or dies as a result of my/their participation in the Event, I, and/or my family or heirs cannot under any circumstances sue Releasees for damages relating to or caused by my injuries or death.
I recognize and acknowledge that Young Life is neither a common carrier or in the business of providing transportation services. In consideration for allowing myself and/or my child to be transported to and from the Event with Young Life, I further reaffirm my assumption of the risks of being transported to and from the Event on behalf of myself and/or my child. I also understand that if circumstances arise where I deem it necessary for I and/or my child to be transported from the Event through a transportation method outside of Young Life, I do so at my own risk and agree to release the Releasees from any and all liability related to such decision. I have read this Waiver, Release and Indemnification Agreement, have asked and received answers to any questions I had concerning its
meaning and execute it freely, without duress, and in full complete understanding of its legal effect, and of the fact that it may affect my legal rights.

I am the parent or legal guardian of the child whose name and signature appear above. I have read and understand this Waiver, Release and Indemnification Agreement, and consent on behalf of the Participant to its terms.
